any questions related to fees,
billing, or insurance. For your
convenience, we also accept
major credit cards.
Before your appointment you
will need to fill out brief
information sheets. You may
also fill out in our online
patient portal or print out the
forms listed below to bring with
you to your visit. Please bring
your glasses, a list of
medications that you take, and
insurance information.
An initial exam is more
comprehensive and time
intensive and usually includes
eye drops to dilate your
pupils. We suggest that you
make arrangements for
someone to drive you home.

is valuable and we make every
effort to remain on schedule.
